Building Use Request Form
In order to insure there are no duplication of events, provide notice to the appropriate staff, and have any set-up/facilities needs met, this form must be completed at least one week in advance of the event. You will be contacted by a member of USD 220 administration to confirm your request. A $100 refundable deposit is required of ALL groups/persons requesting use of the facilities. Any damage to facilities or property become the responsibility of the requesting group. Completion of this form does not automatically grant facility use.  USD 220 events will take priority.
